Before you continue to the application, please review our eligibility requirements above.

Applying does not mean you’ve been accepted. All applications will be reviewed by a case manager, and additional information and documents may be requested.

Applicant must be at least 18 years old as of the date of the application submission.

Applicant must be a legal guardian of a child attending one of the above schools in the target impact zone.

All fields must be completed. Incomplete applications will not be accepted.

Applicant must submit all required documentation & sign the agreement to abide by the program guidelines.
